Psychological factors affect IBS patients interpretation of symptom severity

A patient’s viewpoint of the severity of irritable bowel syndrome (IBS) symptoms can be influenced not only by physical symptoms of IBS but broader psychological problems, according to a new study in http://medicalxpress.com/news/2011-11-psychological-factors-affect-ibs-patients.html, the official journal of the American Gastroenterological Association.

“Clinicians who face pressure to treat patients in a cost-effective manner within tight time constraints and at a satisfactory level are likely to find that patient-reported outcome data can increase their understanding of what patients mean when they describe how they function or feel,” said Jeffrey Lackner, PsyD, of the University at Buffalo School of Medicine and Biomedical Sciences, and lead author of this study. “To maximize the utility of patient-reported outcomes, it is important to know what they measure and what influences patients’ perceptions of their symptoms when gastroenterologists ask them about their symptoms. Our study suggests that irritable bowel syndrome patient-reported outcomes are not simply about gastrointestinal symptoms.”

Patient-reported outcomes (PROs) are used to describe symptoms, inform treatment planning and gauge the benefit of treatments for gastrointestinal disorders, including IBS. In this study, funded by the National Institute of Diabetes and Digestive and Kidney Diseases, researchers explored two different PRO rating scales that measure IBS severity, and identified psychological factors that might bias PRO ratings by affecting how patients interpret symptom severity. They found that a substantial proportion of the variation in the PROs (50 to 55 percent) could be explained by three distinct gastrointestinal (GI) symptoms: pain, bloating and defecation.

Irritable Bowel Syndrome eased by hypnotherapy – Thousands of people with irritable bowel syndrome will be offered hope by new research showing that hypnotherapy eases the conditions in sufferers. Professor Roland Valori, a gastroenterologist at Gloucestershire Royal Hospital, in Gloucester, said heRoland Valorihas used hypnotherapy with success on his patients. In a podcast to launch Frontline Gastroenterology, a new medical journal, he said experience of the first 100 patients treated with hypnotherapy, nine out of ten showed significant improvement in their symptoms. In four out of ten the condition cleared up completely while the remainder said they felt more in control. Irritable bowel syndrome is a long term disorder of the digestive system causing pain, bloating, diarrhoea and constipation. Prof Valori said: “To be frank, I have never looked back.”

Angela is one of the millions of Britons thought to suffer from Irritable Bowel ­Syndrome (IBS) — a problem with gut function which can cause symptoms such as constipation, diarrhoea, painful ­cramping, bloating and flatulence. The symptoms can be so severe in some cases that sufferers have to give up work. It is notoriously difficult to treat and, while the symptoms are undeniably real, tests often reveal no obvious physical cause, explains Dr Julian Stern, who treated Angela at the specialist bowel ­centre at St Mark’s ­Hospital, London.

Thanks largely to the work of Peter Whorwell of Wythenshawe Hospital, Manchester, hypnosis is now acknowledged by the medical profession as an effective treatment for IBS. However, Peters research doesn’t mean that it is universal one size fits all. It is the opinion of NICE that hypnosis is the best treatment for Irritable Bowel Syndrome after diagnosis and medication has failed or partly failed.

This said your doctor will not, unless you are one of the fortunate whose GP has trained as a Hypnotherapist, be able to give you hypnosis as a treatment. Not all ‘lay’ hypnotherapists will be able to help either you should consult a hypnotherapist who specialises and ideally who has had IBS themselves.

Let the Mind Help Tame an Irritable Bowel

If you’ve ever had butterflies in your stomach or an attack of nerves that sent you racing for the bathroom, you already know that the intestinal tract has a mind of its own. The millions who suffer from irritable bowel syndrome, or I.B.S., perhaps know it best. I.B.S., with its symptoms of bloating, abdominal pain, flatulence, diarrhea or constipation or an alternating cycle of the two, can seriously impair the ability to work and enjoy leisure activities. Up to 15 percent of the population is affected, though only half seek medical help. The gut and brain are intimately connected, with more nerve cells in the intestines than in the spinal cord. The gut has been called the body’s second brain, containing 95 percent of the body’s neurotransmitter serotonin and direct nerve connections to the brain.

So it is no surprise that this common disorder of intestinal function has a strong mind-body connection. This does not mean I.B.S. is a psychosomatic condition caused by emotions, but rather that emotional upsets can aggravate symptoms in someone with a hyper-reactive bowel. It also means that learning to minimize stress and emotional disturbances can reduce the symptoms of I.B.S., perhaps more effectively than medications, recent research has indicated. Yet much educational material about this condition underplays the mind-body connection and the vital role that emotional retraining can play in controlling it.

This is perhaps an overreaction to the past when most patients with I.B.S. were told there was nothing physically wrong with them — it was all in their heads. Around 1 in 10 people with IBS could benefit from a gluten free diet. Now, a simple and reliable home test kit can tell you whether you should be gluten free in just 5 minutes.

The importance of an early diagnosis of gluten intolerance (coeliac disease) cannot be over stressed.  Left undiagnosed and untreated, coeliac disease carries a higher risk of developing osteoporosis, infertility and bowel cancer later on in life.

If you do have coeliac disease, a gluten free diet could change your life within weeks… but first, get a diagnosis.

Feeling tired, IBS symptoms, urgency to use the toilet after eating, nausea, mouth ulcers, diarrhoea, bloating, wind, constipation, headaches, depression, bone and joint pain, weight loss, anaemia…. the list goes on and could mean you have IBS but any one of these symptoms could also mean that you have coeliac disease (gluten intolerance).

Unlike IBS, coeliac disease (also spelt celiac) is a risk factor for other diseases such as bowel cancer, infertility and osteoporosis. Early diagnosis is essential. It is estimated that 1 in 10 people originally diagnosed with IBS actually have coeliac disease.

In 2006 and 2007 the home coeliac test was used to test MPs at the House of Commons as part of an awareness raising event by Coeliac UK, a charity providing support and advice for people diagnosed with coeliac disease.

A report in the British Medical Journal  which studied 2690 children and 120 adults showed that the finger prick blood test for coeliac disease was as accurate as hospital laboratory tests (Korponay-Szabo et al. BMJ 2007;335;1244-1247).
